Town Square Family Health Centre is located just two blocks away from KSB Hospital. This where the residents and some faculty members see their patients. We pride ourselves on offering team-based care with support from many different disciplines, including our clinical pharmacist, social worker, chronic care management team and of course our nursing staff and medical assistants.
We do many office procedures at the FHC including skin procedures, joint and headache injections, colposcopy, IUD placement and removal, Nexplanon placement and removal. We have an ultrasound machine and slit lamp in the FHC, and we can do spirometry right in the office.
Our focus is on continuity of care. The goal at the FHC is to have each resident see their own patients at every visit. When that is not possible, we make sure patients are scheduled with someone on that physician’s team.
The Family Health Center was recently recognized as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA.) Only practices that have proven their commitment to a patient-centered approach to care and continued quality improvements can earn this esteemed designation.
